The following Memorandum provides you an opportunity to initiate a Personal Guideline for Advance Planning. Simply check the boxes below, in return we will provide the value of these services and precise options available to you.

It is important to keep in mind that it is helpful to leave some decisions for your family allowing them to participate in the arrangements.



SERVICES:
It is important to note that our services include the transfer from the Place of Death into our care, administration and documentation. Disbursements are not included, however, will be discussed with you upon request.
Family Gathering: A private time scheduled for family to gather for reflection and share support. This gathering is encouraged regardless of any visiting or service.
Public Gathering: A period of time scheduled by the family allowing other family members and friends to pay their respects, offer support and love.
Life Celebration / Funeral Ceremony / Mass of Christian Burial:
Congregation of family and friends at a time and day set by the family at a location designated by the family. This offers an opportunity to reflect on the life and may be personalized to the desire of each family.
Burial Services: Your preference is to be interred in a family plot, already purchased
or one that will be purchased in the future.
Cremation Services: Your preference is to be cremated following the services then
interred, entombed or scattered at a cemetery.
Final Interment:
Who would you like to be involved with your (the) service? ie. Eulogy
What is important to you to have in the Gathering Room & Place of Service that
appropriately reflects your life? Ie. Books on display, gardening equipment, golf
clubs etc.
What is important to you to have be a part of your (the) service? ie. Memorial
Cards, Orders of Service, Dove Release, video tribute etc.
